Program Description

The program is designed to help individuals living with diabetes make informed dietary choices that support blood sugar control, overall health, and long-term disease management. This program provides nutritional education, meal planning guidance, access to diabetes-friendly food supplies, and consultations with dietitians. Through a combination of community outreach and direct support, participants receive practical resources to improve their eating habits and maintain a balanced diet.

To improve nutrition and food accessibility for individuals living with diabetes by providing education, meal planning resources, and access to healthy food options that support effective disease management.

Program Goals & Expected Impact

  • Conduct quarterly nutrition workshops to educate participants on diabetes-friendly diets.
  • Provide personalised meal plans based on dietary needs and local food availability.
  • Distribute monthly diabetes-friendly food packages to at least 2,000 individuals in vulnerable communities.
  • Facilitate one-on-one and group consultations with dietitians, reaching at least 30,000 individuals annually.
  • Support participants in adopting healthier eating habits and improving blood sugar control through practical nutrition guidance.
